HPI: 'Patient is here to discuss participation in our Advanced Primary Care Management Program and qualifies based on the following chronic diseases:'

Assessment & Plan (A&P): 'APCM enrollment was discussed with the patient and included Medicare criteria regarding eligibility, care management, planning and access to the patient's digital care plan, billing and possible monthly co-pay or co-insurance responsibilities, single PCP oversight, as well as their right to withdraw from APCM at anytime. Other related items such as clinical care goals, care team coordination, and social determinants of health were also discussed and these items will continue to be addressed throughout their APCM care plan execution.
The patient has provided verbal consent to participate in APCM and may sign the consent at their earliest convenience.'
